Abstract


The uterine inversion defines itself anatomically as the invagination of fundus uterus bottom "finger of glove" until be able to take out in the vulva. It is a dramatic accident of the placenta delivery and a rare eventuality in countries with low medical entity, this rarity - who can mislead the practitioner - ends in complications that lead maternal death. We present the case of a patient de 30 the years old who had a difficult delivery at home 3 months ago with death newborn in immediate post-partum. She was admitted for small vaginal bleeding associated Burning micturition with preservation of the general states. The gynecological examination had revealed a mass a 5 cm in diameter in the vagina. We made the diagnosis of a chronic puerperal uterine inversion of the third degree and a surgical intervention of vaginal vertical posterior colpohysterotomy by low way in the first one and the second was the colpohysterorraphy by laparotomy.

L'inversion utérine se définit anatomiquement comme l'invagination du fond utérin en ‹‹doigt de gant›› jusqu'à pouvoir au maximum s'extérioriser à la vulve. C'est un accident dramatique de la délivrance et une éventualité rare même dans les pays à faible entité médicale, cette rareté -qui peut induire le praticien en erreur - aboutit à des complications redoutables voire le décès maternel. Nous présentons le cas d'une patiente de30 ans qui a eu un accouchement difficile à domicile 3 mois avant son admission avec décès du nouveau-né au postpartum immédiat. Elle était admise pour des métrorragies minimes associées avec des signes urinaires à type de brulure mictionnelle évoluant dans un contexte de conservation de l'état général. L'examen gynécologique avait objectivé une masse de 5 cm de diamètre environ dans le vagin. Nous avons posé le diagnostic d'une inversion utérine puerpérale chronique du troisième degré (masse friable mal limitée), et une intervention de colpohystérotomie postérieur verticale par voie basse en première et la deuxième était la colpohysterorraphie par laparotomie.
