Abstract


Malaria due by Plasmodium falciparum is the cause of 39.3% of deaths among hospitalized children under 5 in the Democratic Republic of Congo. Today, in addition under five die by malaria as victim of fake drugs or even of auto medication, with the major risk of inducing selection of Plasmodium resistant strains to conventional antimalarial drugs. This study was conducted to determine the knowledge, attitudes and practices on malaria by care providers. A transversal and descriptive survey was conducted in 43. The following results were obtained: 50% of nurses and 50% of physicians were interviewed, 61.1% did not retrained, 38.8% know more than five signs of severe malaria; 84.1% versus 15.9% require laboratory confirmation before treatment; 51% recommend intermittent preventive treatment of pregnant women; 1.7% prescribe chloroquine; 12% prescribe quinine, 8.5% prescribe amodiaquine monotherapy and 20.5% tablet artesunate monotherapy for uncomplicated malaria; quinine is prescribed to 99.2% and 1.7% intra venous artesunate for complicated malaria treatment of and 78.5% did not refere cases beyond their competence. Recycling remains a key concern, anarchy prescribers and non-adherence to national policy providers are barriers to malaria control efforts, we must emphasize the campaigns on education, information and communication for health providers behavior change on case management in the health structures.

En République Démocratique du Congo, le paludisme à Plasmodium falciparum est la cause de 39,3% des décès parmi les enfants de moins de 5 ans hospitalisés. On meurt aujourd'hui de paludisme en étant victime de faux médicaments ou même de l'automédication avec le risque majeur d'induire la sélection des souches plasmodiales résistantes aux antipaludiques usuels. La présente étude a été menée en vue de déterminer des connaissances, attitudes et des pratiques relatives à la lutte antipaludique des prestataires de soins. Une enquête transversale et descriptive était conduite dans 43 structures. Les résultats suivants ont été obtenus : 50% d'infirmiers et 50% de médecins ont été interviewés ; 61,1% n'ont pas bénéficié d'un recyclage ; 38,8% connaissent plus de cinq signes du paludisme grave ; 84,1% versus 15,9% exigent la confirmation biologique avant le traitement ; 51% recommandent le traitement préventif intermittent de la femme enceinte ; 1,7% prescrivent la chloroquine ; 12% prescrivent la quinine ; 8,5% amodiaquine monothérapie et 20,5 artesunate comprimé monothérapie pour le paludisme simple ; la quinine est prescrite à 99,2% et artesunate intra veineux 1,7% pour le traitement des accès pernicieux et 78,5% ne référent pas les cas qui dépassent leur compétence. Le recyclage reste une préoccupation fondamentale, l'anarchie des prescripteurs et la non-adhérence des prestataires à la politique nationale sont des entraves aux efforts de lutte antipaludique, il faut accentuer les campagnes sur l'éducation, l'information et la communication en vue du changement des comportements des prestataires de soins dans les structures de prise en charge des cas.
