Abstract


Endometrial stromal tumors are rare and are less than 5% of uterine tumors. They are composed of cells closely resembling endometrial stromal cells in the proliferative phase. They are classified into two categories: the stromal nodules, benign, and endometrial stromal sarcomas, including endometrial stromal sarcoma (low grade) and undifferentiated endometrial sarcoma (formerly high grade).
The diagnosis is most often does retrospectively after histological analysis of the surgical specimen. The classical treatment is abdominal hysterectomy with bilateral oophorectomy. The adjuvant treatment are not yet established and the optimal treatment of this entity is still not clear. We report the case of a patient of 46 years, admitted for pelvic pain with abdomino-pelvic mass
Ultrasound and MRI have found a polymyomatous uterus with interstitial body mass with a compression effect on the endometrium. The patient underwent total hysterectomy with bilateral oophorectomy, Histology with additional immunohistochemical concluded endometrial stromal sarcoma of low grade, stage IB, hormone receptor-negative.
Through this rare case and with a literature review, we focus on the epidemiological, diagnostic, therapeutic and prognostic of that entity.

Les tumeurs du stroma endométrial sont rares et constituent moins de 5 % des tumeurs utérines. Elles sont composées de cellules ressemblant étroitement aux cellules du stroma endométrial en phase proliférative. On les classe en deux catégories : les nodules stromals, bénins, et les sarcomes du stroma endométrial, comprenant le sarcome du stroma endométrial (de bas grade) et le sarcome endométrial indifférencié (anciennement de haut grade) . Le diagnostic est le plus souvent porté rétrospectivement après l'analyse histologique de la pièce opératoire. Le traitement classique est l'hystérectomie abdominale avec annexectomie bilatérale. Les indications concernant le traitement adjuvant ne sont pas encore établies et le traitement optimal de cette entité n'est toujours pas clair. Nous rapportons le cas d'une patiente de 46ans, admise pour douleur pelvienne avec une masse abdomino-pelvienne. L'échographie et l'IRM ont trouvé un utérus polymyomateux avec une masse corporelle interstitielle avec un effet de compression sur l'endomètre. La patiente a bénéficié d'une hystérectomie totale avec annexectomie bilatérale L'étude histologique avec complément immunohistochimique a conclu à un sarcome du stroma endométrial de bas grade, stade IB, récepteurs hormonaux négatif.
A travers ce cas rare et à la lumière d'une revue de la littérature, nous insistons sur les caractéristiques épidémiologiques, diagnostiques, thérapeutiques et pronostiques de cette entité.
