Abstract


Background: Uterine rupture achieves a solution of continuity of the thickness of the wall of the womb. It can be spontaneous or provoked. It can occur during work (most often) or during pregnancy. In all cases, although this occurs in the third trimester of pregnancy, uterine rupture can also occur before this period of gestation, the second trimester of pregnancy. Case: A 29-year-old woman, gesture 8, par 8, with a history of in utero death and uterine incision presented a hemoperitoneum picture of a 26-year-old pregnancy. She consulted 8 hours after the sudden onset of severe abdominal pain and metrorrhagia. Uterine rupture was observed during laparotomy, and adequate maternal management was performed. Conclusion: The occurrence of uterine rupture should be suspected before any hemoperitoneum, hemodynamic instability or metrorrhagia, even before the third trimester of pregnancy. Early diagnosis could improve the fetal-maternal prognosis.

Contexte : La rupture utérine réalise une solution de continuité de l’épaisseur de la paroi de l'utérus. Elle peut être spontanée ou provoquée. Elle peut survenir pendant le travail (le plus souvent) ou pendant la grossesse. Dans tous les cas, bien que cela s’observe au troisième trimestre de la grossesse, une rupture utérine peut aussi se réaliser avant cette période de gestation, soit au deuxième trimestre de la grossesse. Cas : Une femme de 29 ans, geste 8, pare 8, avec antécédents de morts in utero et d’incision utérine a présenté un tableau d’hemopéritoine sur une grossesse de 25 SA. Elle a consulté 8h après l’installation brutale d’une vive douleur abdominale et de métrorragies. Une rupture utérine a été constatée lors de la laparotomie, et une prise en charge maternelle adéquate a été réalisée. Conclusion : La survenue d’une rupture utérine devra été suspectée devant tout tableau d’hemopéritoine, d’instabilité hémodynamique ou de métrorragie, même avant le troisième trimestre de la grossesse. Un diagnostic précoce pourrait améliorer le pronostic fœto- maternel.
