Abstract


Background: Hypothyroidism is among the most common endocrine disease; it is often associated with dyslipidemia which increases cardiovascular risk. This study was conducted to study the prevalence and profile of dyslipidemia during primary hypothyroidism and justify screening for dyslipidemia during hypothyroidism.Methods: Retrospective study, in the biochemistry department CHU Ibn Rochd of Casablanca from 2019 to March 2020 involving all patients who received a lipid assessment and a TSH (Thyroid Stimulating Hormone) assay in the biochemistry laboratory. TSH was estimated by an immunoenzymatic method: chemiluminescence. The LDL-assayed (Low Density Lipoprotein), HDL (High Density Lipoprotein), TG (Triglycerides), CT (Total Cholesterol) parameters were estimated by an enzymatic colorimetric method, the LDL-calculated was calculated by the friedwald formula.Results: These were 1046 patients: 66.73% Women, 33.23% Men (sex ratio M / F = 0.49). The mean age was 41.66 years ± 13.86. The mean TSH value was 23.09 mIU / L. Dyslipidemia was noted in 91.78% of cases.The prevalences of, hypoHDLemia, hypercholesterolemia, hypertriglyceridemia, hyperLDLemia, were respectively: 82.12%; 32.5%; 23.32%; 10.8%.The correlation between TSH and LDL was negative (P> 0.05) while it was positive with TG (P> 0.05), HDL (P <0.05) and CT (P> 0.05).Conclusion: Hypothyroid patients show a significant increase in serum lipids. This atherogenic lipid profile should be systematically sought when monitoring patients with hypothyroidism to prevent cardiovascular complications.

Introduction: L’hypothyroïdie est parmi les pathologies endocrines les plus fréquentes, elle est associée souvent à une dyslipidémie augmentant ainsi le risque cardiovasculaire. Cette étude a été menée pour étudier la prévalence et le profil de la dyslipidémie au cours de l’hypothyroïdie primaire, justifier le dépistage des dyslipidémies au cours de l’hypothyroïdie.Matériel Et Méthodes: Étude rétrospective, menée dans le service de biochimie CHU Ibn Rochd de Casablanca du janvier 2019 à mars 2020 intéressant tous les patients ayant bénéficié d’un bilan lipidique et d’un dosage de TSH (Thyroid Stimulating Hormone) au sein du laboratoire de biochimie. La TSH a été dosé par une méthode immunoenzymatique: la chimiluminescence. Les paramètres LDL-dosé (Low density lipoprotein), HDL (high density lipoprotein), TG (Triglycérides), CT (Cholestérol Total) ont été dosés par méthode enzymatique colorimétrique, le LDL-calculé a été calculé par la formule de friedwald.Résultats: Il s’agissait de 1046 patients: 66,73% Femmes, 33,23% Hommes (sex-ratio H/F=0,49). L’âge moyen était 41,66 ans±13,86. La valeur moyenne de TSH était à 23,09 mUI/L. Une dyslipidémie était notée dans 91,78% des cas.Les prévalences de, l'hypoHDLémie, l’hypercholestérolémie, l’hypertriglycéridémie, l'hyperLDLémie, étaient respectivement: 82.12%; 32,5%; 23.32%; 10.8%. La corrélation entre TSH et le LDL était négative (P>0.05) alors qu’elle était positive avec les TG (P>0.05), HDL (P<0.05) et CT (P>0.05).Conclusion: Les patients hypothyroïdiens présentent une augmentation significative des lipides sériques. Ce profil lipidique athérogène doit être recherché systématiquement lors du suivi des patients en hypothyroïdie pour prévenir les complications cardiovasculaires.
