 Relevance of prescriptions for urgent biochemical analyzes at Casablanca Ibn Rochd University Hospital

Abstract


Introduction: As a consequence of the discovery of numerous abnormalities in the prescription of biochemistry emergency parameters, the objective of this work was to establish an inventory of the emergency prescriptive practices within hospital departments of Ibn Rochd university hospital of Casablanca, for possible corrective actions. Material and methods: A prospective transversal descriptive study was carried out on all the biochemical assessments prescribed during the guards (nights and weekends) and done in the laboratory of biochemistry of UH-Ibn Rochd of Casablanca, from June 1st to October 31st, 2015. The statistical analysis was done on Excel. Results: During the study period, we received 9020 biochemical assessments with 73216 parameters. About 12% of these assessments were prescribed by non-emergency departments (and 88% by emergency departments). We found that non-emergency parameters were prescribed with a relative high frequency such as lipid status (triglycerides accounted for 0.48% of prescriptions (n = 352), total cholesterol 0.45% (n = 330) and LDL-cholesterol and HDL-cholesterol 0.3% (n = 220). In addition to this, 90% of the prescriptions were sent without clinical information. Discussion and conclusion: This study revealed dysfunctions concerning the prescription of emergency biochemical assessments. For this purpose, a clinical-biological collaboration is necessary to implement corrective actions such as the drafting of prescription procedures of emergency assessments.

Introduction: Suite à la constatation de nombreuses anomalies de la prescription des paramètres de biochimie d’urgence, l’objectif de ce travail était d’établir un état des lieux des pratiques prescriptionnelles d’urgence au sein des services hospitaliers du CHU Ibn Rochd de Casablanca, pour des éventuelles actions correctives. Matériel et méthodes: Etude transversale descriptive prospective portant sur tous les bilans biochimiques prescrits pendant les gardes (nuits et les week-ends) et réalisés au sein du laboratoire de biochimie du CHU-Ibn Rochd de Casablanca, du 1er juin au 31 Octobre 2015. L’analyse statistique a été réalisée sur Excel. Résultats: Au cours de la période de l’étude, nous avons reçu 9020 bilans biochimiques avec 73216 paramètres. Environ 12% de ces bilans ont été prescrits par des services qualifiés comme «Non-urgents» (et 88% par les services qualifiés comme urgents). Nous avons constaté que des paramètres considérés comme non urgents étaient prescrits avec une fréquence relativement élevée comme le bilan lipidique (les triglycérides représentaient 0,48% des prescriptions (n=352), le cholestérol total 0,45% (n=330) et LDL-cholestérol et HDL-cholestérol 0,3% (n=220). Par ailleurs, 90% des bilans reçus ont été acheminés sans renseignements cliniques. Discussion et conclusion: Cette étude a mis en évidence des dysfonctionnements concernant la prescription de bilans biochimiques urgents. A cet effet, une collaboration clinicobiologique s’avère nécessaire pour mettre en place des actions correctives comme la rédaction de procédures de la prescription et du traitement des bilans d’urgence.
