Abstract


The cornual pregnancy is a rare entity that represents almost 2% of ectopic pregnancies. Its diagnosis is almost always based on laparoscopy. The risk of rupture with severe bleeding remains high. The surgical treatment is often conventionally cornual radical resection, although cases medical methotrexate have been described.
The objective of this work is to describe a rare form of ectopic pregnancy and expose its diagnostic features, management and prognosis.
Patient and method: We report the case of a cornual pregnancy discovery intraoperatively in a patient of 27 years, admitted for acute pelvic pain with bleeding in the first quarter with pelvic ultrasound: empty uterus, endometrium 10mm, with an average abundance of peritoneal effusion. Rate of beta-hcg 2017. Given the strong suspiscion of ectopic pregnancy the patient was admitted immediately block with discovery of a right cornual pregnancy ruptured.
Discussion and Conclusion: Interstitial pregnancy ectopic pregnancy is a rare, difficult to diagnose which may involve life and maternal fertility prognosis. Taking conventional charge is surgical. In subsequent pregnancies the clinician wary risk of recurrence of interstitial pregnancy and uterine rupture.

La grossesse cornuale : entité rare qui représente près de 2% des grossesses extra-utérines. Son diagnostic repose presque toujours sur la cœlioscopie. Les risques de rupture avec hémorragie importante restent élevés. Le traitement est classiquement chirurgical par résection cornuale, quoique des cas de traitement médical par méthotrexate aient été décrits.
L'objectif de ce travail est de décrire une forme rare de grossesse extra-utérine et d'exposer ses particularités diagnostique, prise en charge et pronostiques.
Patient et méthode: Nous rapportons le cas d'une grossesse cornuale découverte en per opératoire chez une patiente de 27ans, admise pour algies pelviennes aigues avec à l'échographie pelvienne : utérus vide, endomètre de 10mm et un épanchement péritonéal de moyenne abondance. Taux de béta-hcg à 2017. Devant la forte suspiscion de grossesse extra-utérine la patiente a été admise immédiatement au bloc avec découverte d'une grossesse cornuale droite rompue.
Discussion et Conclusion: La grossesse cornuale est une grossesse ectopique rare, de diagnostic difficile, qui peut mettre en jeu le pronostic vital maternel. La prise en charge classique est chirurgicale. Lors des grossesses ultérieures le clinicien se méfiera des risques de récidive de grossesse interstitielle et de rupture utérine.
