Abstract


The objective of this study is to describe and investigate the circumstances of an epidemic infections outbreak of multidrug-resistant Acinetobacter baumannii in pediatric intensive care unit. Between August and October 2012, where ten strains of Acinetobacter baumannii have been isolated. Blood cultures and protected distal bronchial samples were the main sites of isolation. The study of antibiotic resistance helps to identify two phenotypical clones confirmed by the genotypic study using the pulsed field gel electrophoresis. The audit on respect of the hygiene precautions by healthcare workers revealed a large lack. The line adopted following this event was the establishment of corrective measures in order to stop this epidemic. Reinforcing the hygiene measures and staff training were successful in ending the outbreak without having to close the unit. The emergence of strains of multidrug-resistant Acinetobacter baumannii infections outbreak in the intensive care units seems very alarming and requires the development of an effective and permanent strategy against healthcare associated infections.

L'émergence d'infections à Acinetobacter baumannii multirésistants aux antibiotiques reste préoccupante en raison du risque d'impasse thérapeutique. L’objectif de cette étude est de décrire une épidémie à Acinetobacter baumannii multi résistant au service de réanimation. Entre août et octobre 2012, 10 souches d’Acinetobacter baumannii ont été isolées chez plusieurs enfants hospitalisés au sein du service de réanimation pédiatrique. Les hémocultures et les prélèvements bronchiques distaux protégés ont constitué les principaux sites d’isolement. L’étude phénotypique basée sur l’antibiogramme a permis d’identifier au sein des isolats, 2 clones différents. L’exploration génotypique par électrophorèse en champ pulsé des 10 souches isolées, a confirmé la présence de 2 profils probablement clonaux et épidémiques circulants durant cette période. L’audit du respect des règles d’hygiène réalisé au sein du service auprès du personnel de soins a révélé une grande insuffisance dans l’observance de la désinfection des mains par les solutions hydro alcooliques. La conduite à tenir adoptée était l’établissement de mesures correctives dans le but de stopper cette épidémie à un germe multi résistant transmis par manuportage. Le renforcement des mesures d'hygiène et la sensibilisation du personnel ont permis de mettre fin à l'épidémie sans fermeture du service. L’apparition et la diffusion de souches d’Acinetobacter baumannii multi-résistant au sein du service de réanimation pédiatrique semble très alarmante, et nécessite l’élaboration d’une stratégie efficace de lutte contre l’infection nosocomiale.
